Participation Requirements

If any student wants to try-out for any sport at BCMS, he/she must have a physical completed by a doctor and the insurance packet filled out. All paperwork MUST be completed and turned into the coach 3 days prior to try-outs. If you need to print up the physical form, we have attached it below.
